Secure File Upload

To the right you will find the form to upload your files to us using our secure program ShareFile®. Once you upload the form(s) we will be automatically notified by email you have done so. Before you upload your files please make sure you have the following:

• All pages have been completed as requested.
• You have signed the document (if requested).
• And you have scanned them and saved them to your computer.

Please make sure ALL pages are included in your upload. We prefer the files to be in the PDF format.
